Checklist for Student Accessibility Services
Student Accessibility Services (SAS) is the program responsible for determining and providing appropriate academic accommodations for students with disabilities. In order to determine a student's eligibility and to provide services, the following steps are recommended:

- Apply to 91重口 through the 91重口 Enrollment Services Office.

- Sign up for an orientation.

- Register for classes. Instructions are on the 91重口 Enrollment Services webpage.

- Apply for Student Accessibility Services. New students must fill out a and provide documentation at least two weeks before the accommodations are needed. A SAS staff member will contact you for an intake appointment.

- For your intake appointment, please provide:
- Most recent Individualized Education Plan (IEP) or 504 Plan.
- Current psycho-educational report and/or medical verification of disability provided by a licensed professional.
- A letter of diagnosis and recommended accommodations from a counselor.
- A letter of current enrollment in a substance use support program.

Make An Appointment
- Make an appointment to meet with a Student Accessibility Specialist:

- You can make an appointment for yourself .
- You can make an appointment by contacting a SAS staff member by phone or email.
- You can make an appointment by coming to the High-Tech Lab, Room E106.

- Complete intake procedures.
- Build an Academic Accommodation Plan.
